For this piece, I developed a school-based community health program that was focused on reducing the rates of repeat adolescent pregnancy among high school students. My program plan includes: a mission statement, goals and objectives, logic model, needs assessment of my priority population, description of the program and the theory used to create it, budget justification, marketing strategy, and evaluation of the plan. From the assignment, I also learned about the importance of using theory when establishing a base for an intervention, as well as the significance that community engagement can have on the success of a program.
